Welcome to the 2026 Summer Art Studio Program at Silvermine Arts Center!

So that your child is ready to go on their first day of camp please review and submit all forms at least 2 weeks before the start of your childs camp date.

We REQUIRE parents to submit contact information for each child attending by filling all the fields in the form below. If you have more than one child attending camp you must fill out and submit a seperate form for each.

We also require a medical form to be submited via email if they relate to your child.  Medical Authorization Forms are required for children with life-threatening allergies or health conditions, signed by both a doctor and a parent.  The medical form should be emailed to schoolregistrar@silvermineart.org.
